A Befriending Care Co-Ordinator will provide one to one companionship and support to adults feeling isolated and lonely, provide company, support and encouragement to people accessing community services and activities. They will be a caring listener for people who may not have others they feel comfortable talking to.

We will expect the Befriending Coordinator to help recruit, train, and supervise volunteers who will with your support become befrienders and buddies to clients within the service.

This role will require you to be able to work with, and understand the roles of, a variety of different people working in the practice and across the PCN including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, physician associates, paramedics, health and wellbeing coaches, podiatrists, occupational therapists and pharmacy technicians.

They may be given a caseload of identified patients and be required to ensure that their changing needs are addressed by taking into account local priorities, health inequalities and/or population health management risk stratification.

Befriender Role Profile
- Undertake assessments over the phone and face to face with the elderly to find out more about their hobbies, interests, and personal information to enable us to find a successful befriending match and update referrers of our progress.
- Play an integral role in the matching process connecting volunteers and scheme members for friendship and company.
- Attend and participate at weekly team meetings and other staff meeting and check ins.
- Troubleshoot / respond to challenges as they arise in partnership with the Lead Health & Wellbeing Coach and Line Manager.
- Respond to safeguarding enquiries and undertake all the necessary follow up under the supervision of the Line Manager and Lead Health & Wellbeing Coach.
- Undertake risk assessments.
- Maintain accurate records and other databases/spreadsheets.
- Be responsible for communicating sensitive information and maintaining confidentiality in accordance with the PCN’s policies and procedures.
- Present the ethos, aims and purposes of the PCN Befriending service to individuals and their families, volunteers, professionals and within agency settings, housing schemes, conferences, and events.
- Contribute to the development of operational documents and processes as required.
- Undertake training as required.
- Attend and contribute towards the planning and delivery of group befriending, seasonal events and community outreach projects.
- Contribute towards and participate in internal and external research and monitoring.
- Conform to all policies, procedures and guidelines laid down by the PCN.
- Perform such other duties as may reasonably be required.

It is essential that, if the legal requirements are to be met and the trust of our patients is to be retained, all staff protect patient information and provide a confidential service.
