**Clinical Care Coordinator - Band 6**

**Warminster/Westbury Integrated Community Team / Avenue Surgery Warminster**

**37.5 Hours per week - Permanent**

Community Healthcare is changing across Wiltshire and beyond and its one of the most exciting transformations the sector has seen. The aim is not only to serve the local people its to offer outstanding services that exceed their expectations.

As part of a patient centred initiative Wiltshire health and Care LLP in partnership with GP practice teams have recruited Care Coordinators as ambassadors for healthcare, championing excellence and integration. Care Coordinators work with GP practice teams to coordinate care for their vulnerable patients. The role involves acting as an advocate for patients and their families, signposting to other appropriate services and liaising with providers including health, social care and independent organisations including charities and the voluntary sector. The Care Coordinators with Primary Health Care Teams supporting with avoidance of hospital admissions and reducing in-patient length of stay to ensure that patients receive the requisite care in the most appropriate setting. Care co-ordinators also contribute to planning, implementing and evaluating care plans for patients on behalf of the Primary Health Care Team.
