Forest Heath PCN has been formed between Brandon Medical Practice, Forest Surgery, The Surgery - Lakenheath, The Market Cross Surgery, Oakfield Surgery, Orchard House Surgery, The Reynard Surgery and Rookery Medical Centre serving our combined 74,000 patients in Newmarket, Mildenhall, Brandon and the surrounding villages.

**JOB TITLE: Visiting Paramedic/EMERGENCY CARE PRACTITIONER**

**REPORTS TO: THE PARTNERS (Clinically)**

**Job summary**:
The post-holder will be responsible for the autonomous clinical assessment and treatment of patients, ensuring the highest standards of care for all registered and temporary patients. The role will involve the assessment, diagnosis, treatment or referral of patients presenting with undifferentiated and undiagnosed conditions. The post-holder will work within the Emergency Care Practitioner’s level of competency. The ECP may be required to respond to referrals and requests from various sources, including:

- Patients
- Care homes
- General Practitioners
- 111
- Out of Hours services

**Clinical responsibilities**:

- To provide assessment, diagnosis and treatment as first point of contact by attending to patients according to patients’ needs, either in the Practice or in **patient’s homes**:

- To provide safe and effective clinical practice within own competence boundaries, and acknowledge limitations
- To support and continue the development of an on the day emergency patient service in the Practice
- **The role involves visits, ward rounds, triage and assessment of Nursing/Care home patients, as well as housebound patients**:

- To accurately triage and prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history
- To evaluate clinical information from examination and history taking, and initiate appropriate treatment and/or referral
- To undertake and interpret diagnostics and take appropriate action to manage patient care, whilst distinguishing between abnormal and normal findings in order to relate them to a possible diagnosis. To use broad clinical guidelines (NICE etc.) relating them to individual cases, providing assistance and feedback to the organisation and others on how these should be interpreted
- To signpost appropriately to other services in the identified healthcare pathway.
- To refer patients to appropriate care settings, within locally agreed healthcare pathways
- To discharge or treat, as appropriate and within the scope of practice
- To maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse events.
- To advise and support members of the public on healthy lifestyles, health promotion and disease prevention.
- To provide clinical advice to other members of the team where appropriate

**Other responsibilities within the organisation**:

- Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety
- A commitment to life-long learning and audit to ensure evidence-based best practice
- Contributing to evaluation/audit and clinical standard setting within the organisation
- Contributing to the development of computer-based patient records
- Contributing to the summarising of patient records and read-coding patient data
- Attending training and events organised by the practice or other agencies, where appropriate
