Key tasks and responsibilities To work (with MDT support) within own professional boundaries and competencies. Take responsibility for developing own clinical competence, leadership, and reflective practice skills Delivery (or skills to be developed,) flu vaccinations, B12, ECGs, venepuncture, and other relevant clinical tasks as required by the PCN, in line with the competencies of the role Supporting patients, families and carers by providing relevant information on diagnosis, signposting patients to further information or referral to social prescribing link workers etc. Performing and recording clinical observations and consultations on EMIS WEB such as blood pressure, temperature, respirations, and pulse Recording clinical information accurately and consistently across the PCN practices through recognised templates for data entry. Discussing and sharing information with registered nurses and or wider clinical team on patients health conditions, activities, and responses Developing an understanding the delivery of care to our patients with complex conditions such as dementia, mental health conditions, and learning disabilities Working within Practice and in the Community, including patient's home, care/residential home.
Embed the core NMC values and expectations of care, compassions, courage, communication, commitment, and competence into everyday practice. Communication Internally : Individual Practice & PCN Teams, Clinical Director/ PCN Development Manager Externally : Patients, Relatives and Carers, Community & Secondary Care Teams, Social services Attend meetings and provide/deliver documentation in support of planning pathway strategies in delivery of care to patient cohorts and achieving targets. This Job Description/Specification and duties may be amended following consultation with PCN management, to facilitate the development of the role and PCN objectives. All interested parties/ personnel should be prepared to accept additional, or surrender existing duties, to enable the efficient running of WDPCN.

Ability to develop effective and appropriate relationships with people, their families, carers and colleagues across supporting organisations Future opportunities to support, supervise, assess and act as a role model to nursing associate students, other learners and health care support workers as required within the clinical setting. Engage will colleagues and help influence new enhanced ways of working with particular emphasis on digital development Confidentiality & Security In performing the duties, the post-holder may have access to confidential information. Staff are therefore required to maintain appropriate standards of confidentiality relating to all aspects of their work, colleagues, practice/PCN business, service users and third-party information and must adhere to policies and procedures relating to confidentiality and the protection of personal and sensitive data.

On appointment, the postholder will complete an induction programme with their nominated contact(s) within the practices of the PCN and, if applicable, their line manager hoisting their contract, Including: Mandatory training Taking responsibility for own development, learning and performance. Maintaining a record of own personal and/or professional development Supervision in accordance with good practice guidelines Participation in appraisal process. Thereafter, participating in any training programme implemented as part of this employment.
