**Job Title: Care Co-ordinator - Early Interventions Team**
**Trust Location: Crossgate House**
**Pay Rates: Days £19.10 per hour**
**Saturdays and nights £24.83 per hour**
**Sundays and bank holidays £30.56 per hour**
**Contract length**: On going - bank shifts
**Working hours**: Mon-Fri 9-5

**Job duties**:
- To manage a caseload of people with complex mental health and social care needs, using evidence-based practice to assess, plan, implement and evaluate interventions and document these in accordance with local and national standards and guidance.
- To have care-coordination responsibilities for an agreed number of clients on enhanced Care Programme Approach (CPA) within the team.
- To participate in the assessment of people referred to the team with possible first episode psychosis.
- To participate in the operational systems and the development of the Early Intervention team.
- To provide leadership for junior staff, multi-disciplinary support staff and students through formal supervision, education and appraisal.
**Clinical**:
- To manage a caseload of people age 14-35 with first episode psychosis taking on the role of care-coordinator as agreed.
- To plan and implement psychosocial interventions with the client and family following appropriate training
- To reassess, evaluate and measure outcomes to ensure effectiveness of the intervention.
- To participate regularly in the team assessment procedures via the duty systems or assessment clinics.
- To provide consultation and advice to schools, colleges and other community agencies to facilitate early identification and referral of young people with possible psychosis.
- To monitor the effects and side effects of medications prescribed for the treatment of psychosis.
- To participate within the directorate AMHP rota if a trained AMHP
**You’ll have the following qualification/experience**:
- RMN or Social worker
- Post registration qualification in a clinically orientated area
- Psychosocial interventions qualification
- Qualification in leadership or management
- Post registration experience.
- Recent experience working in either adult mental health or CAMHS.
- Significant experience of work in a range of mental health settings
- Leadership and first line management
- Experience of working with people with early onset psychosis and their Families.
- Working in a community setting and lone working
- Lived experience of Mental health issues or a caring role of someone with mental health issues.
- Professional Registration
