Digital & Transformation Lead

We are looking to recruit to a Digital & Transformation Lead to work within our Goldstone Primary Care Network team in Hove. Goldstone Primary Care Network is a network of 3 practices - Charter Medical Centre, Trinity Medical Centre and WellBN Wellbeing - with a combined list size of over 78,000 patients.

The post will be based at Charter Medical practice, with a focus on this practice to begin with.

The post holder will plan, coordinate and directly support delivery of digital and transformation activity, with a view to improving patient access, staff and patient satisfaction, optimising the use of technology and improving the efficiency and sustainability of general practice services

The postholder will work with a diverse range of people from different cultural and social backgrounds. The ability to work confidently and effectively in a varied, and sometimes challenging environment is essential.

Salary depending on experience

£48,526 - 54,619

**Job summary**

Meet government set targets and key performance As Digital and Transformation lead you would be expected to support increased access to care for patients, by supporting the adoption and/or optimisation of new technology and other initiatives to improve the care offer, and enabling Practice/ PCN staff to work more effectively to support the sustainability of general practice services.

Key responsibilities:
a. Improve adoption and/or optimisation of new technology to enhance patient access and experience and increase Practice/PCN productivity

b. Build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients (including shared appointments between practices to aid delivery of enhanced access)

c. Lead an improvement approach to change including building capability for quality improvement within the Practices /PCN and system wide approaches to problem solving

d. Review and improve the Practices/PCN’s digital maturity

e. Use data, and improve data quality, to:

- Understand demand, capacity and activity
- indicators
- Drive improvements in patient experiences of access and staff experience at work.
- Support population health management
- Support understanding of the type and intensity of support/training needs of the PCN and coordinate this support, including through OD programmes
- Facilitate clinically led innovation and the effective adoption of improvement initiatives, including integrated working at neighbourhood and/or place level to improve access to services for patients.

**Project Management**
- Deliver significant project that support the Practices/PCN and lead to improvements in health outcomes for the patient population.
- Manage the whole lifecycle of a programme of system change.
- Support organisational change and the uptake of initiative that support and embed excellence within the health system.
- Review project reports and risk logs for projects, advising individual project managers on escalation process and supporting delivery of projects where there may be significant barriers to delivery.
- Manage the whole lifecycle of a programme of work containing multiple projects, reporting on progress and managing risks and issues, escalating where necessary to the project board.
- Provide expertise of best practice change and project management methodologies.
- Ensure realisation of benefits for customers through planning and designing and that project outputs meet programme requirements.
- Support organisational change and the uptake of initiatives that support excellence.
- Demonstrate flexibility and support new programmes and projects where necessary.

**Service improvements**
- Responsible for the delivery of, robust, information systems to ensure that all project documentation is stored and used effectively (PID, dashboards, highlight reports, risk and issues logs etc).
- Develop programme and project management tools, report and templates that are transferable between projects and can be used for the whole project lifetime, from design to delivery.
- To act as a champion for patients and their interests and involve the public and patients in the policy development and decision-making.
- To ensure all public and patient contact with the office is of highest professional standard.
- To embed patient and public involvement at all levels of decision making.
- Be a champion for best practice, learning from experience and from others, supporting the spread of innovation within the organisation.

**Analysis and Judgement**
- Interrogate and analyse, sometimes highly complex, information to support the delivery of clear project reporting and project outcomes.
- Write and submit regular high-level project reports for the programme board showing progress against milestones, status, resource requirements, issues, risks and dependencies, making recommendations where a range of options may exist.
