Would you like the chance to provide crucial support to Children and Young People (CYP) with a Learning Disability (LD) and/or Autism (A) (up to 25 years) at risk of admission to a mental health hospital?

As a LD/A Keyworker, you will have passion and commitment when delivering quality support within the family home, placement, or inpatient setting. You will work as part of a team to prevent - where possible - CYP from requiring an inpatient admission or placement.

Are you an Expert by Experience or a Professional (nurse, social worker, occupational therapist, speech & language therapist, psychological therapist, assistant psychologist, or youth worker) and have experience of providing support to CYP with a learning disability and/or autism.

The 0-25 year’s keyworker model aims to support reduction of the number of children in crisis and mental health admissions, supporting timely assessments/formulations, providing children and families with the support that they need at the right time, and improved transitions & quality of life, resulting in happy children and families. A key function of the service will be to empower families and support them with navigating barriers across multiple organisations (NHS, Local Authority, Voluntary sector, etc) to effectively access the right support from the system. Our keyworking model is about a set of functions introduced to the system via the keyworking role, in addition to the current roles in place to support CYP and their families.

- To ensure that local systems are responsive to meeting their needs in a holistic and joined up way
- To improve outcomes for children and young people with learning disabilities, autism or both.

- Facilitate the provision of emotional and behavioural support and informed advice to the CYP and their family/carers based on knowledge and experience of working with CYP and families/carers with complex needs.
- Troubleshoot on behalf of the CYP and their family/carers, where required, to ensure the necessary support is in place to enable them to remain within, or discharged into the community and promote joined up, consistent care, constructively challenging the status quo, where required.
- Create and encourage a culture which prioritises keeping CYP within the family home and local community by persuasively communicating with a range of teams, services and staff at different levels of seniority.
