An exciting opportunity has arisen to join our service as an Associate Practitioner. You will be working independently and accountably as part of the Nursing team and wider Multi-Disciplinary Team: offering physical health care assessments alongside psych-social support/education.

You will act as a role model to other colleagues within the Service. You will work collaboratively with key partner organisations, service users, clinicians and other practitioners with the collaborative community based services.

To be responsible along with other members of the team for the implementation of all care aspects of individual care planning.

Attend to the physical and psychological needs of the service users under the care of the service.

Undertake physical health care checks of service users and act on the findings.

To take a lead role for physical health monitoring, on behalf of the service.

Produce accurate and timely documentation / records on all clients in line with relevant codes and Trust Policy.

Attend the weekly MDT meeting.

Act as key worker (Under the supervision of a registered clinician).

Under the supervision of a registered clinician take part in assessing, planning and reviewing care.

Transport and store medical equipment in line with Trust Policy.

Record all service user related activity information into Paris.

Deliver smoking cessation training.

Be an Infection Control Link person (to support 1stlevel nurse).
