The post-holder will hold a substantive contract within the NHS through Lancashire Teaching Hospitals NHS trust (LTHTr) and honorary contracts with Alder Hey Children’s Hospital. The hospice element of the post provides medical palliative care support to in patients across the Children’s Hospices of Lancashire and South Cumbria (Derian House Children’s Hospice, Brian House Children’s Hospice). That would include direct clinical care of patients, especially at the end of life or where symptom control was difficult, in collaboration with GPs and other community and acute paediatric consultants. It also includes sensitive and timely management of discussions in relation to plans for the end of life in partnership with other professionals, including the family support team.

Specialist paediatric palliative care provision in Lancashire and South Cumbria -The post holder will provide direct clinical care to children and young people with palliative care needs in Lancashire and South Cumbria and will be supported by lead paediatricians in each of the four acute hospital trusts. Where appropriate, care will be provided within a shared care model of working. The post-holder will participate in the weekly multi-disciplinary team meetings held in Alder Hey Children’s Hospital (usually by videoconference), and will see patients across the region as needed at their home, hospice or on acute paediatric hospital wards.

In the hospice, the post holder will provide medical advice to children and their families requiring palliative and end of life care as the clinical lead for the multidisciplinary children’s palliative care team. This will involve management of children in a variety of locations in the community including patient’s homes, schools and within the hospice. It will include support for end of life care at home if the family wishes.
