Strategic Health Facilitator

We are seeking an energetic, highly motivated, and experienced registered Learning Disability nurse who has extensive learning disability and mental health experience to work within our Community Learning Disability Team based on the Isle of Wight.
You will be expected to manage your caseload effectively to improve the experience for the adults who have a diagnosis of Learning Disability and /or autism. The ability to communicate effectively and share information appropriately within a multi-agency context is also essential.
You will need to have excellent communication and interpersonal skills, be flexible and capable of working independently, as well as part of the wider Adult CLDT.
The Adult CLDT is Island wide, clinics/visits will be held in various locations across the Island, therefore the post holder must be a driver.
The post is full time Monday to Friday.
We are able to offer relocation packages or support with travelling to the Isle of Wight.
**The post holder will**:
Improve the Primary and Secondary care health experience of people with a learning disability.
Contribute to the design and delivery of a Directorate and Locality strategic health facilitation work plan.
Provide advice and support to others concerning health facilitation in the pursuit of improving access to mainstream health services for people with a learning disability.
Be a member of the multi-disciplinary Learning Disability team.
Contribute to effective multidisciplinary working within the Learning Disability team and build effective liaison with community, primary, other secondary and tertiary services and agencies to meet individual need and service development.
Carry out all relevant forms of care without direct supervision, and will be required to demonstrate procedures to, and supervise qualified and unqualified staff.
Mentor, supervise and teach qualified and unqualified staff including students
Ensure that systems are used to incorporate any adopted models of quality assurance and governance into everyday practice.
Participate in the recruitment, deployment and management of staff.
