**Main Objectives of Role**:To lead a team of care staff to care and support people living in the care home to live their lives the way they want to and how they want to. Ensuring that they support people to achieve their outcomes and what matters to them.

**Experience of writing and updating care plans is required along with on-call out of hours duties.**

**Key Tasks**
- Embedding a “team work” ethos among care staff at all times
- Directing and mentoring the care workforce; allocating tasks and duties and planning staff breaks
- Ensuring that they and the care workforce are engaging with people in a meaningful and person-centred way when supporting people; putting them at the heart of everything
- Registering people moving into the home with local GP surgeries and ensuring that their prescriptions are requested in a timely manner
- Ensuring that medication is administered and signed for in line with the relevant medication policy and procedure and that the medication trolley is kept clean and tidy before handing over on each shift
- Assisting the Deputy/ Home Manager with the ordering, booking of monthly medication
- Ensuring any acute medications are received and recorded efficiently
- Assist Deputy / Home Manager with DOLS Assessment.
- Supporting any visiting health professionals or social workers by giving them relevant information about a person living in the home
- Facilitating and conducting “handovers” in between shifts that communicate to other staff any changes in needs, changes to medication, accidents or incidents
- Supporting the deputy manager/manager with carrying out medication audits and the booking in and returning of all medication
- Monitoring the health and wellbeing of people living in the home and contacting health professionals, social services, family/representatives and recording this information in care plans and handover communication
- Ensuring that care staff are checking the texture of food; soft or puree diet and thickened fluids before supporting people who require these measures to and drink
- Answering the telephone and signposting the calls to the relevant people
- Managing the service on weekends when there is no management on site but contacting management of needed out of hours
- Greeting families and /or visitors to the home and ensuring they have any questions they may have answered; carrying out viewings of the home
- Dealing with any emergencies effectively and professionally
- To attend to developing situations in relation to health and well
- being of the residents
- Complete and attend to any accidents/ incidents ensuring all documentation is completed
- Attending any supervisions and appraisals as arranged by the manager/deputy manager
- Supporting and assisting people with their daily living tasks; bathing, showering, dressing, eating and drinking, oral hygiene, mobilising around the home, managing continence, managing skin viability/repositioning
- Reading and following care plans and risk assessments and encouraging the care workforce to do the same
- Reading the company’s policies and procedures and adhering to them; encouraging the care workforce to do the same
- Reporting to the deputy manager/manager any changes to physical or mental health efficiently and effectively and recording these concerns
- Supporting new staff to complete induction
- Ensuring the cleanliness and safe environment of the home
- Deal with any staff situations as they arise
- Escalate any issues to the Deputy/ Home Manager

- . Obtain any required samples
- . Monitor the whereabouts and safety of the residents
- . Complete the weekly ward round with the GP and ensure the information is shared between staff/ Manager's and complete all instructions from GP
- . Manage the visiting file and completing the attached forms/meet visiting families to ensure smooth visit is had and as booked
- . Inform the staff of who has a visit that day and ensure they are ready to receive their visit
- . Any other tasks as needed
- Reporting any serious accidents or incidents swiftly to the deputy manager/manager and completing the necessary paperwork. Managing such less serious events day to day using own initiative
- Checking that the care workforce are recording day and night logs in detail and with accuracy
- Dealing with and reporting any safeguarding concerns to the deputy manager/manager
- Ensuring that the care workforce is following infection control procedures at all times and ensuring the PPE is worn; and also adhere to this practice.
