Trainee Advanced Care Practitioner Renal

This is an exciting opportunity to join the fantastic renal team at Salford Care Organisation as a Trainee Advance Care Practitioner within the Rapid Access Service, whilst studying your fully funded apprenticeship to gain a MSc in Advanced Practice.

The overall training programme is 24+ month long working towards the four pillars of advanced practice. Throughout the programme you will be supported with protected study time/blended learning/on the job learning.

On completion of the tACP programme the post will progress to an 8a grade. The programme has also been accredited by Health Education England.

We are looking to recruit a Trainee ACP who has clinical experience in renal care and are ready to take the next step in their clinical career.
You will work within a dynamic multi-disciplinary team, under the supervision of Renal Consultants and will join an established Renal ACP team to enhance the renal outpatient experience through the delivery of excellent patient-focused care. The post will also include cross site working at one of our established Kidney Care Centres.
You must have proven experience in your clinical specialty. There will be ample opportunity to explore your own interests and support innovations in outpatient renal service delivery.

As a ACP, you will be responsible for leading The Rapid Access service alongside Medical and Nursing Colleagues to ensure that our service users’ needs are met through the delivery of high-quality care.
- The post holder will practice under supervision as a trainee Advanced Clinical Practitioner (tACP) to provide high quality patient centred care. This will utilise advanced assessment, diagnostic and treatment skills within an agreed scope of practice.
- The Advanced Nurse Practitioner within the Rapid Access Service will be responsible for their own clinics, referrals into the service and follow ups.
- You will have the opportunity to learn and develop practical procedural skills such as tunneled venous dialysis catheter insertions.
- You will be involved in service development, driving, and supporting innovative changes in the management of chronic kidney disease across primary and secondary care.
- You will be supporting several clinics such as hypertension, Multi Morbidity, Cardio-renal, chronic kidney disease optimisation clinic
- You should have an appetite and drive to be involved in research and quality improvement projects.
