**Advert for Apprentice Advanced Clinical Practitioner (ACP)**: Older Peoples Mental Health Service, based at The Meadows Community Hospital - Band 7

An exciting opportunity has arisen for a highly motivated and enthusiastic apprentice Advanced Clinical Practitioner (ACP) to work within our Older Peoples Mental Health Services, based along-side our established memory services. This a new post within Stockport Older People’s Service.

The successful applicant will study on the Master’s programme at Salford University alongside your clinical role to develop your skills in Advanced Clinical Practice. In your role as an apprentice ACP you will work clinically within the Memory Assessment and wider dementia services in line with local, strategic & NICE guidance. The apprentice ACP will act as a professional resource for the MDT, patients and carers within the scope of their practice and competence.

The post holder will be an active senior leader for the Older Peoples Service, offering leadership and guidance and the wider team and be involved in service development work. You will provide clinical assessments and work closely with the team to give clinical direction and oversight, provide supervision and practice development opportunities for colleagues.

You will also offer support to colleagues and Service Users within the wider Older People’s Service encouraging and leading MDT discussions for complex cases and clinical issues. This would lead to development of individual and person centred management, risk formulation and treatment plans.

You will participate and contribute to the trusts Inter-Professional Learning agenda. Supervision will be provided by a dedicated medical supervisor, service leads within the trust and attending the Trust ACP forum.
