Overview:

We have an exciting opportunity for a **Recovery Support Coordinator** who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs. As a recovery support coordinator with Change Grow Live you’ll work with people who use our services and help support them into transitioning back into local communities and developing their social and local resources. This will support long term recovery.

With this role you’ll be working as part of Health and Wellbeing team, based in Wirral. The team supports people with re-integration back into local communities after treatment, and focuses on supporting people to build social and local resources, that builds a foundation of recovery capital.

No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key.

**Where**: Wirral

**Hours**: 37.5 per week

**Salary**: £27,449.53 - £31,529.41 Pro Rata dependant on experience

**Responsibilities**:
**About the role**:

- Supporting service users in Aftercare/Recovery Support through their remaining time in treatment with the service.
- Providing screening, assessment, and recovery planning and onward referral
- Reducing drug and alcohol related harm to service users and the wider community
- Promoting carer, service user and community involvement
- Work with prescribing clinicians to support substitute prescribing regimes to improve social functioning.
- Aftercare care coordinator overseeing assigned groups of service users, providing: 1-1s; structured group work; asset mapping; recovery planning (using motivational interviewing techniques) and reviews of care.

**About you**:

- Have a strong understanding of substance misuse issues and experience of working within a similar field
- Experience of working with groups/pods and managing group dynamics, such as conflict or challenging behaviour.
- Be an excellent communicator, both verbal and written with good IT skills
- Proactive and able to work on own initiative with excellent time management and prioritising skills
- Have a good working knowledge of mental health interventions, services, and good practice
- Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
- Ability to manage change successfully in a way that prioritises the needs of service users
- Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
