Senior Diabetes Specialist Nurse

This is a fantastic opportunity for an enthusiastic and experienced Diabetes Specialist Nurse.
This is a new service working with Primary Care to improve diabetes outcomes through education and joint working.
You will be jointly responsible for managing the service and team and there will be opportunities for you to shape the service going forwards. This will include the setting up of nurse led and locality clinics. The service will have the sup port of a Diabetes Consultant.
Although based at the Diabetes Centre at the King Edward VII Hospital in Windsor you will work primarily with Primary Care Healthcare Professionals in practice.
Informal visits welcome
We are looking for an experienced Diabetes Specialist Nurse as areas of service provision will include.
Support to General Practice with the management of people with diabetes.
Education and training to Healthcare Professionals involved in providing diabetes care in Primary Care
Promote close working relationships between primary and the specialist service.
Support best practice and high standards of diabetes care through:
Positive leadership
Excellent communication skills
Expert knowledge of diabetes
Role model and mentor ing
