Icc Clinician- Wallingford

We are looking to appoint experienced clinicians across Oxfordshire to work within our new Intensive Community Care pathway alongside the End of Life Matrons, Falls Specialist Nurses and UCR Specialist Practitioners. This is a highly varied role with a high acuity of patients that is both challenging and rewarding. This position would be suitable to many disciplines such as Physiotherapists, Occupational Therapists, Paramedics and Nurses as you will be provided with the appropriate training and competency framework to support your development into this role.

At a national level, the NHS Long Term Plan has made a commitment to boost 'out of hospital' care, supporting people to 'age well' in their own home. Our new intensive community care pathway is expanding our existing ambulatory care services into a more robust, safe, and effective alternative to acute hospital admission, tailored for the needs of older and frailer people, operating 365 days a year.
