As part of your role as a health support worker within the Connect adult eating disorder team will be to support service users to improve their confidence and motivation to implement practical and social changes in their day to day lives linked to their goals for treatment.

You will be part of a multi disciplinary approach to service users care, holding your own caseload with support and direction from the MDT. Your role will play a pivotal part in supporting service users on their treatment journey with Connect.

You will be tasked with supporting service users with food shopping, meal preparation, planning, portioning, social eating, meaningful activity and clothes shopping amongst other duties.
As part of your role, there will be many opportunities to support the team in things such as service development, project work and other areas of interest linked to your continuing development.

The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country. 1. JOB SUMMARY AND WORKLOAD MANAGEMENT
- To provide a high standard of care as planned and directed by multi-disciplinary team (MDT), to an allocated group of service users with diagnosis of eating disorders in the community, as part of a home intervention programme.
- Work as an effective member within a MDT whilst following relevant aspects of prescribed /individual care plans, with a focus on the practical aspects of treatment.
- Able to use initiative, managing and prioritising workload/caseload whilst following the job role and its defined responsibilities.
- To provide input/feedback regarding service user’s progress and adherence with treatment plans with wider MDT
- Communicate effectively with the multi-disciplinary team, service users, carers and other external agencies.

2. PRINCIPLE DUTIES AND AREAS OF RESPONSIBILITY
- Provide the care that has been agreed on by the MDT in service users’ homes and the wider community, whilst offering support and encouragement to service users, giving feedback as necessary.
- With the support of your supervisor and Allied health professional (AHP) Clinical Lead to have a shared responsibility for a caseload.
- Work autonomously, competently, and safely and utilise supervision as well as the guidance of colleagues as required.
- For the post holder to be aware of safeguarding and risk issues, including physical health concerns and report them to a senior member of the team as a priority.
- Work within an agreed treatment plan, dealing with routine and urgent clinical

problems/interactions, face to face, via video calls or on the telephone, seeking direction from the MDT.
- Engaging in therapeutic activities with service users, without supervision, including food shopping, meal preparation, eating out activities, engaging in meaningful activity and supporting where appropriate with clothes shopping. To also undertake such other duties as may be determined from time to time within the general scope of the post.
- Develop rapport with service users based on appropriate and transparent boundaries.
