The GP Out of Hours based at Macclesfield Hospital, is looking to recruit 2x Band 6 Triage Practitioners who are looking to work on a Part Time basis. We are looking for the post holders to predominantly work weekends.

The GP Out of Hours service provides Urgent health care and advice during Nights, Evenings, Weekends and Bank Holidays when patients own doctors surgeries are closed, and we work closely with NHS 111 Service and NWAS in providing this care. We also operate an Acute Visiting Service weekdays enabling direct access from paramedics and GP surgeries as part of the NHS England's admission avoidance scheme.

As one of our Triage Nurses, you will clinically assess and prioritise all patients who are referred to the service by NHS 111, NWAS and other HCP’s, either by Telephone Triage or Face to Face Triage (of patients streamed through from ED). You will provide self care advice appropriate to the patients needs, or arrange a face to face consultation with a GP at one of our bases or at home, when necessary.

You will be a Registered Nurse or Paramedic with 5 years post registration experience in relevant disciplines e.g Practice Nursing, Emergency Department, NHS 111, Health Visiting, Community Nursing and have evidence of continuing professional development.

**Hours per week**: 7.50

To provide clinical triage as an integral part of the GP Out of Hours Service for the population of Eastern Cheshire CCG.

To work as part of a team of Triage Nurses/ Clinicians, GP’s, Call Handlers and Drivers within the GP Out of Hours service based at Macclesfield District General Hospital.

The post holder will utilize clinical triage and assessment skills, whilst working within the Scope of Professional Practice and adhering to local policies/guidelines.

The post holder will manage and prioritize direct referrals to the service, from NHS111, ED, NWAS and other HCP’s with a professional and courteous manner at all times.

The post holder will be responsible for the operational delivery of a safe and effective service.
