The Responsive Workforce Team (RWT) is part of the DCHS workforce strategy aimed at ensuring that quality people are available to deliver a quality service in line with the DCHS Way.

The RWT will provide high quality professional nursing care in a range of locations and specialties’.

Team members will need to be flexible about location, working patterns and environment as they will be required to regularly face new challenges in both hospital and community settings.

Team members will receive training and support to enable working in both hospital and community settings.

Team Members will be required to assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient’s privacy and dignity.

To provide advice and support to junior staff in the delivery of high quality care and actively support the local manager in the management of the health care environment.

The role requires specialist knowledge across a range of procedures underpinned by theory to enable them to work in a variety of settings.

We have exciting opportunities in substantive positions for highly motivated, enthusiastic nurses to provide cover across our inpatient wards and in community settings with flexibility to provide specialist support for longer term gaps and meet service demand.

You will work within teams within both community hospitals and community services to offer support and to assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient’s privacy and dignity. This role is an exciting opportunity, you must have two years or more band 5 experience and hold a full driving license, having access to a car.

KEY DUTIES TASKS AND RESPONSIBILITIES
- To undertake clinical assignments across a range of locations covering gaps in rotas. For example to cover periods of service redesign, longer term sickness absence, seasonal pressures.
- To support the local manager in the day to day operational management, taking on agreed tasks. For example day to day co-ordination of healthcare assistants, link for specific key developments, appraisals for health care assistants.
- Be responsible for the assessment, implementation of treatment and the development of programmes of care for designated patients and provide feedback / escalate discussion regarding meeting patient need through supervision/monitoring processes.
