Dementia and Frailty Care Coordinator

This new exciting opportunity has arisen to appoint an enthusiastic and self-motivated Dementia and Frailty Care Coordinator to make a difference to the care and support our patients receive in Bramhall and Cheadle Hulme Primary Care Network (BaCH PCN.)

BaCH PCN comprises the 5 GP Practices in the vicinity, and covers a population of 57,700.

The Dementia and Frailty Care Coordinator will work as part of the PCN Clinical team and will be the link between the patient, the 5 GP Practices and the wider multidisciplinary team around the patient. This will help support people affected by dementia and/or frailty by completing holistic assessments and creating person-centered support plans. This will involve visiting people in their own homes, and possibly other community settings.

**Main Duties and Responsibilities**
- Establish, develop, and maintain excellent partnership working relationships with GPs and other key stakeholders across Bramhall and Cheadle Hulme PCN (Primary Care Network).
- Ensure ongoing support is given to people affected by dementia and or frailty and their carers. Empowering them to prepare for future planning and remaining as independent for as long as possible.
- Identify each individual’s health, wellbeing and social support needs using a strengths-based approach.
- Attend MDTs (Multidisciplinary Team Meeting) to contribute to discussion, support networks and provide updates on the people being supported.
- To contribute and work towards reducing crisis, carer breakdown and hospital admissions.
- Uphold the highest standards of data protection, record keeping, impact monitoring and safeguarding to protect the people we support.
- To ensure regular evaluations are completed to monitor and review the impact of the service.
- Engage in regular supervision and develop professional and personal skills through annual appraisals and completion of competencies.
- Contribute towards the development of the service and be flexible in approach to necessary changes within the service.
