An opportunity has arisen to work in an exciting integrated model for the delivery of patient care in Dorset County Hospital’s Acute Hospital at Home (AHAH) service. This service proactively manages and monitors a defined caseload of patients who have complex care needs in the short term, on a multi-disciplinary basis in their own home.

We are looking for a dynamic experienced generic Nurse/Therapy Band 4 Practitioner, to work in our multi-disciplinary team, ensuring a patient focused and individualised approach. You will need Nursing & Therapy skills equivalent to band 3 and in-date venipuncture & cannulation skills. (see PS for requirements) You will have acute and community experience and have experience of effective multi-disciplinary team working in its widest sense.

Car Driver Essential.

The post holder will work closely with the trained staff in the day to day provision of patient care. Undertake observations on stable patients once competency attained delegated by registered nurse. To work with patients in the daily practice of rehabilitation programmes, enabling time and practice to promote independence in activities essential for discharge. This may include working towards continence, increasing oral intake, independence administering medications, giving opportunities for communication practice, making own meals, independence in self-care and mobility.
