You will be a qualified professional with experience of supporting people and their families, preferably in the community. You will demonstrate a strong understanding of your role as a Care Coordinator/Assessor and join our dedicated multi-disciplinary team working closely with a range of partners to provide appropriate community interventions for those individuals referred to the team.

You will enjoy plenty of opportunities for joint working, training, and a sound supervision structure.
- Undertaking urgent/routine initial assessments on behalf of the team
- Attending regular MDT meetings and liaising with colleagues from other services (both internally and externally such as CRHTT, Adult Social Care and third sector organisations)
- Supporting and managing a caseload undertaking face to face visits in the community setting including home visits
- Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders
- Working alongside Lead Care Professionals and supporting those currently under the Care Programme Approach
- Participating in the duty worker rota (Monday to Friday 09:00am to 17:00pm)
- Medication administration (if qualified to do so) with opportunities to enrol in local university training programmes in medication management
