An exciting opportunity has become available for a Band 6 Nurse or Practitioner to join the South Lakes Care Home Education and Support Service (CHESS) on a 12 month contract, maternity cover, for 17.5 hours per week. The team are looking for a dynamic colleague who shares our passion for ensuring that individuals enjoy great care and health regardless of age and diagnosis.
This person must be able to work as a member of a strong multi-disciplinary and a very progressive team.

We are looking for a forward thinking, caring individual who has excellent leadership qualities and experience of working with mental health difficulties relating to the ageing process and people who have a dementia diagnosis. Experience in a care home setting is desirable.
One of the main roles of this post is to supervise, educate and support staff, manage referrals and to communicate team needs.
Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
