**Position**: Extra Care Team Leader

**Reporting to**: Extra Care Scheme Manager

**Role**:
As a Care Team Leader, you will be monitoring and supervising the work of the care team. You will monitor and review service delivery, carry out assessments to meet legislation and complete all appropriate paperwork for care planning and Service User files. You will be responsible for all residents within the building during out of hours if you are on shift, responding to any residents requiring support.

**Duties**:

- Promoting quality care and support to service users, to enhance existing levels of functioning and quality of life
- Regularly monitoring service delivery
- Performing spot checks and conducting reviews
- Conducting risk assessments for new service users
- Getting involved in the day to day care
- Responsible for the maintenance of the building at times
- Respond appropriately to anyone needing immediate attention with the Scheme
- Take part in activities
- Complete staff 1:1's
- Producing comprehensive and detailed monthly reports
- Undertaking other duties from time to time

**Requirements**:

- Ideally NVQ/SVQ Level 2 qualified
- You must have at least 2 years care work experience
- This role will involve an element of on-call duties
