An exciting opportunity has arisen to join the Surgery Division at The Hampshire Hospitals NHS Foundation Trust as a Cancer Pathway Navigator. We are seeking to recruit a highly motivated, confident and dynamic individual who will be a central point of contact for patients on a rapid diagnostic, suspicious of cancer pathway; helping to ensure the individual needs of the patient are supported, whilst facilitating a seamless, coordinated pathway delivery, such as through escalation of requirements to relevant departments, supporting a better, more personalised diagnostic experience for patients.

We are look for a lung Cancer Pathway Navigator.

The post holder will identify and resolve barriers to care, working flexibly and responsively to unlock bottlenecks and delays. You will therefore be required to work autonomously and have strong organisation, communication and negotiation skills, as well as commitment and compassion for patients and their quality of care and experience.
- To screen trust wide 2ww clinic lists to ensure relevant information is gathered in preparation to patient investigations
- From 2WW cancer clinic, engage with the patients, to establish a rapport and act as a single point of contact for the patient.
- Manage any tracking system or databases for the pathway within the Trust and ensure a contemporaneous version is available and accessible to colleagues’
- To ensure the referral information, patient letter and any relevant clinical information is available for fist clinical vetting/assessment, including performance status/frailty score and recent blood tests.
- Work closely with the administrative services across sites (including booking team), diagnostic services and clinical team to ensure next actions are regularly updated and completed.
- Identify patients who are at risk of breaching the pathway milestones and standards and escalate appropriately to enable early and prompt actions to prevent breaches.
- We actively identify situations where dates of diagnostic tests and treatment may not be scheduled appropriately, exercising own judgement and be responsible for ensuring appointments/procedures are expedited as necessary with guidance from line manager
- Have a full understanding of trust policies and processes and Cancer Wait Time Guidance relating to cancer pathways and navigate patients through their diagnostics and treatment pathways within these principles
