Discharge Coordinator

The RM is a world leading Cancer Centre specialising in diagnosis treatment care education and research. It plays a vital role in pioneering and improving cancer. An exciting opportunity has arisen for an experienced registered nurse or a qualified social worker with extensive discharge planning experience to join the Discharge Team at The Royal Marsden Chelsea site.

You will work with the MDT to facilitate the effective flow and discharge of patients work alongside the ward nursing staff to coordinate discharge plans Identify ‘internal’ discharge delays and work with the MDT to ‘unblock’ these delays to enable the timely discharge of patients.

You will support the wards with discharge planning, including specialist assessments under the National Framework for NHS Continuing Healthcare and Funded nursing care.

You will need to have the ability to work flexibly and independently but also enjoy the experience of being part of a wide multidisciplinary team working with clinical and non-clinical teams.

A broad overview of responsibilities includes providing support to and working extensively with multi-disciplinary and multi-agency staff to co-ordinate complex discharge planning for those patients who require on-going care following discharge from hospital, including monitoring of Delayed Transfers of Care.

The post holder will work a part of the Discharge Team to facilitate a timely, effective, and safe discharge of patients from the acute setting to the community. The post holder will be expected to work with NHS patients and Private Care patients as required in order achieve best outcomes for patients, optimise hospital bed utilisation and reduce length of stay.

The will assist the multi-professional team by giving support and advice and acting as a resource for all aspects of discharge planning. This will include NHS funded continuing healthcare, where their role will be to support the ward to complete the documentation that accurately reflects the patient’s needs on discharge.

The post holder will also play an important role in the on-going education of all appropriate staff to ensure they have the necessary knowledge and skills necessary to plan and implement safe and timely discharges.

Excellent communication and collaboration with colleagues working in hospitals, community, and social and health services is required in order to promote a seamless discharge/transfer for all patients.

To provide support, advice, information and education to ward staff and multidisciplinary teams to facilitate a safe and timely discharge back to the community.

To undertake all aspect of clinical duties as an autonomous practitioner.

The post holder will support nursing staff in obtaining a documented discharge care plan, discharge destination and estimated date of discharge for all inpatients.

The post holder will be able to demonstrate knowledge of the legal framework under and duties relating to hospital discharge and other legislation relating to discharge planning and the reimbursement process.
