Urology Fds Practitioner

The post holder will be an autonomous practitioner proactively managing a caseload of patients presenting through the Suspected Prostate Cancer referral service. The CNS will be expected to deliver nurse-led clinics and supporting patients on the 28 day FDS pathway.
The post holder will be an experienced Urology, Cancer or Surgical Nurse. The role will require excellent communication skills and dedication to improve and develop services that positively impact patient care and experience.
To have an up to date understanding of cancer wait time targets, attend the weekly PTL meetings and work with the specialty cancer lead and Trust cancer team to ensure all targets are consistently met and improve the pathway if required.
The post holder will triage, assess and refer patients on the suspected pathway for the appropriate investigation, to be the key contact for their patients through this process. To be accountable for referring patients within the time frames set within the 28 day pathway and communicate effectively with the patient and other members of the multi-disciplinary team including the General Practitioner.
Ability to assess a patient’s medical fitness and performance status in order to decide whether a patient referred from primary care needs immediate diagnostics, a period of monitoring within secondary care, or discharge back to primary care.
Work within the existing Cancer Nurse Specialist team, Urological team and Cancer management team, providing clinical excellence for patients with a possible Urological cancer. They will also be expected to participate in other activities carried out by the CNS team such as living well events, holistic needs assessments as required. They will develop the skills required to support and advise all patients with a diagnosis of a Urological cancer.
To be able to undertake extended roles in line with patient needs and professional training including carrying out local anaesthetic transperineal biopsies. (LATPB)
