We have an exciting opportunity for you to join our Ongoing Support Team as a Social Care Practitioner.
**LIVE** WORK **SUPPORT**
Our Ongoing Support Teams support adults with issues faced due to a disability, health condition or social circumstances. We have expertise across a range of areas, for example, in working alongside people living with dementia (including increasing numbers of ‘younger adults’), complex neurological conditions, substance misuse, end of life support, physical disability, mental health, homelessness and learning difficulties.
We focus on prevention and strength-based working. Staff support people to maintain their independence through use of equipment, information, professional advice, and guidance as well as providing formal care and support where required.
The teams also support carers/families within their informal caring roles and works in partnership with other organisations, including primary and secondary health care, mental health services, substance misuse services, private providers, voluntary sector organisations and others to provide coordinated support for customers and/or carers.
**About the role**
As a Social Care Practitioner, you will provide person centred, outcome focused assessments, to identify the appropriate support required to enable our customers to make positive changes in their lives.
You will promote customer independence, choice, and well-being in the community, using universal services and support where appropriate.
Staff are required to be adaptable and able, due to the vastly different types of need they will come across throughout each day, to use skills and experience to work in a person-centred way. Skills include completing Care Act Assessments, complex moving and handling skills, minor and major adaptation work**,** assessing fluctuating mental capacity, working with people at significant risk, crisis management, and working with people as part of safeguarding enquiries.
This is a challenging role, and you will be supported by the Leadership Team, Experienced Occupational Therapists, Social Workers as well as highly experienced peer Social Care Practitioners.
**About you**
You will hold a level 4 in Health and Social Care, equivalent or the ability to work towards this qualification, and have experience of working in a health or social care setting and have excellent communication skills.
The work is fast-paced, and you will need to have excellent communication skills and will be able to work collaboratively with multi professional teams.
The role requires the use of a computer-based recording system therefore skills in IT are essential, and knowledge of Liquid Logic would also be an advantage.
