We have a great opportunity for full-time band 6 nurses to work in Intensive Support Element of the Dementia & Older People’s Mental Health Team based in Haywards Heath, West Sussex. Intensive Support operates an 8am - 7pm shift pattern, with a 1-hr break, Monday to Fridays, and 9am - 5pm at weekends and bank holidays. There may be on occasions a need to change this shift pattern to 9-5pm; 7 days a week at short notice to meet any changes that may occur in the service.

You will be part of a highly-skilled and supportive multi-disciplinary team. You will receive regular managerial and clinical supervision. This is a challenging but rewarding work as we receive positive feedback from our service users and carers. Sussex Partnership NHS Foundation Trust has recently achieved Outstanding from CQC for the care it provides.

The post holder will be expected to travel across the geographical areas of Mid Sussex and Crawley and Horsham areas covered by the team, so the ability to travel to remote areas is essential.

Main duties of the job

The role of this post is to actively engage those requiring an increased level of support with the aim of keeping people within their home and maintaining their wellbeing and independence as far as possible. We are looking for individuals who are dynamic, compassionate, good communicator who are able to demonstrate a holistic and person-centred approach to their work.

You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies. We follow a multi-disciplinary approach and within that context you will manage a caseload of older adults as a team.

Detailed job description and main responsibilities

Clinical

To contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence based/client centred principles to assess, plan, implement and evaluate interventions, which will be delivered in the Dementia and Older Adults Mental Health Service.

To provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway

To provide risk screening and assessment as well as risk management planning

To contribute to the maintenance and development of the Service

To regularly share and supervise students on practice placement, and junior staff as appropriate

You will need to be caring, compassionate and a good communicator, with a commitment to improving the lives of our service users through creative, evidence-based practice. As a mental health practitioner, you will be responsible for working collaboratively with service users and their carers to create care plans that meet their needs and maximise their independence.
