We are looking for a pharmacy technician to join us in Wallasey Wellbeing PCN. You will join an experienced team that includes GPs, Nurses, Health & Wellbeing Coaches, Care Coordinators, Social Prescribers & Physiotherapists to provide clinical pharmacy services to our local population.

You will support the PCN with the delivery of safe and effective systems for medicines optimisation, reducing medicines waste and maximising patient outcomes.

You will contribute to new ways of working and using the skills that your role can bring to evolving PCN services.

You will be responsible for promoting good practice in line with clinical and operational guidelines and will support delivery of specific medicines projects, for example:

- Compliance reviews and supporting the SMR process
- Discharge medicines reconciliation
- High risk drug management

On a day-to-day basis you will spend time within the PCN hub and in GP practices to answer queries from patients, GPs and other services such as care homes. You will be supported to build strong relationships with community pharmacy and trust pharmacy colleagues.

Ultimately your role will support our patients to help them get the most out of their medicines and to play an active part in their own healthcare decisions.

As part of the WUTH Integrated Pharmacy Service that supports three PCNs within Wirral, this role is based within Wallasey Wellbeing PCN. There are opportunities to work with colleagues from the other PCN teams as well as having access to the Acute Trust specialists which helps you develop and better understand the full patient journey.
