Cheshire East Council is looking for enthusiastic and committed individuals to join our community adult social care teams as social care assessors.
You will have a varied caseload supporting adults with care and support needs and their carers. You will join a multi-disciplinary team of social workers, social care assessors, occupational therapists and local area coordinators, with a mixture of home and office-based working. We also have a vacancy for a Local Area Coordinator. In this role you will be linking individuals in with third sector and community resources. You will receive support and training to help with your professional development.
We have a number of full and part time vacancies across the south of the authority, in the Congleton, Nantwich and Sandbach areas.

As a social care assessor, you will be working alongside other social care colleagues in a community team. You will be using a Strength Based approach to carry out Care Act assessments and develop support plans for people with care and support needs. You will develop knowledge of local community resources and work effectively with partnership organisations.
You will have experience of working with vulnerable people and their carers in a Social Care or Health setting. Good communication skills are essential and the ability to manage a caseload.
Within the framework of legislation, agreed Council Policies and Procedures, to work with and on behalf of vulnerable people by completing the required needs-based assessments, arranging a full range of services and care managing a case load of clients and carers.
