Adferiad Recovery delivers a new, flexible, and co-ordinated response to the exceptional circumstances faced by people with co-occurring mental health and substance misuse conditions and related issues. We harness the talents and experience of our founding and long-established leading Welsh Charities CAIS, Hafal, and WCADA in their fields, enabling our projects to meet the needs of our most vulnerable with a single, unified, and comprehensive approach.

**This role**:
We have an exciting opportunity available in our Pembrokeshire Housing Services to work as part of team improving the lives of people with serious mental illness. This Pembrokeshire service has been developed in partnership with Hywel Dda, Pembrokeshire Housing Support Grant Team and Pembrokeshire County Council, in line with our commitment to principles of empowerment, self-management and recovery.

This Supported Accommodation project involves a 24 hour shared supported accommodation to six occupants who are diagnosed as experiencing a serious mental illness and have been assessed as needing high levels of support to help with their recovery. The aim of the staff team is to encourage and enable clients to reach a greater level of independence over a reasonable period, and then to facilitate their move on to more independent living in the community.

The service is staffed 7 days a week, 24 hours per day with on-site waking nights and sleep-in cover. Staff have the Rota 4 - 6 weeks in advance.

**Main responsibilities**:
Provide a variety of person centred, tailored interventions that support individuals with complex needs to find and sustain accommodation and develop their independent living skills.

Supporting and assisting occupants with the following:

- Feeling secure within their home
- Increasing independent living skills
- Using appliances safely and maintaining reasonable domestic standards
- Personal finance, budgeting, and accessing benefits.
- Mental and physical healthcare
- Self-management of medication
- Attending appointments
- Visitors and their access to individual’s property
- Building and maintaining relationships with neighbours, families, friends, professionals
- Developing skills and interests
- Enhance their social skills and participate in their community.

Support the Service Manager and Seniors to ensure that individual needs assessments, risk assessments, support plans are all up to date and to offer practical advice and advocacy to service users.

Liaise with relevant services, organisations, and agencies with regards to accessing additional support for service users.

Practice homeless prevention, tenancy sustainment, Psychologically Informed Approaches, Solution Focus Practice, harm reduction, and co-production.

**Person Specification**:

- There are no specific qualifications but there must be evidence of commitment to personal and career development relevant to the post.
- NVQ/QCF Competencies in Health and Social Care Level 2 is desirable, but all staff would be required to hold or complete this as part of their employment.
- We are looking for someone with experience of providing care and support services to people with a serious mental illness or within a social care setting.
- _OR_ A significant experience of personally using or caring for someone using mental health services in a serious mental health setting.

- You must have a full UK Driver's Licence and access to a vehicle for work purposes to be considered for this role.
