**Staying Well Worker**

**Based at Hanson Lane Enterprise Centre**

**22 hours per week**

**Salary £26,008.46** **per annum (actual pro rata £15,258.30)**

**Fixed term until March 2024**

**Key responsibilities**:

- To help adults’ access activities within their community which meet their agreed health and wellbeing needs.
- To support the Care Closer to home model of care by supporting people to maintain independence, self-care and avoid unnecessary hospital admissions
- Effectively generate and manage caseloads of referrals from a variety of sources
- Experience of working with adults from a wide range of backgrounds
- Excellent communication skills
- A genuine interest in working with vulnerable and isolated people
- Level 2 Literacy
- Experience of working within a community setting and/or with people from disadvantaged groups
- A full UK driving licence and use of car for work.
