- Participate in on-call rota (approximately 1 in every 4 weeks, £100 a week when on-call)
**The Role**:
Our Bath Supported Housing service primarily caters to individuals with a recent history of rough sleeping who may not always meet the priority needs criteria for social housing. The project comprises five properties, with a total of approximately 30 clients, offering a variety of support levels with an emphasis on medium to high support.
Our clients often navigate complex life circumstances, which may involve past or ongoing experiences of trauma, mental health challenges, substance misuse, domestic abuse, and interactions with the criminal justice system. As a Supported Housing Worker, your role will be to establish trusting and empowering relationships with clients, offering person-centred support to a caseload of up to 10 individuals. Your objective will be to assist clients in recognising their strengths and aspirations, fostering their skills, confidence, and overall well-being. This support aims to help them sustain their accommodation and prepare for a successful transition, preventing a return to homelessness or life on the streets.
**Responsibilities include**:
- Empower a caseload of clients so they can successfully maintain their accommodation and prepare for a positive move-on
- Implement and monitor client support plans and risk assessments
- Give clients advice, and information and provide an enabling service so they can be signposted to, and access appropriate services
- Work flexibly as part of a team to ensure service coverage and that contractual obligations and people's needs are met
- Maintain complete, accurate and up-to-date client records using our client record system
**Qualifications / Requirements**:
- Experience working with vulnerable adults and/or people with complex needs
- Ability to undertake assessment and planning for people with complex needs which include taking a lead on multi-agency working and partnerships
