What would you be doing at HomeLink?*

Working closely with the hospital staff you will facilitate the effective flow and discharge of patients from hospital into our community teams. Responsible for coordinating discharge plans (identifying and resolving internal discharge delays)
You will need a good working knowledge of identifying care needs and their documentation as you will be giving support and advise for all aspects of discharge of patients.

**What would we expect from you?**

Complete a comprehensive holistic clinical assessment of potential patients reviewing their care requirements to determine their suitability for transition to the community setting, including our virtual ward and early supported discharge pathways; this face to face review is underpinned by a detailed review of patient medical history, close liaison and clinical handover with the Trust MDT (Multidisciplinary Team and is documented in real time via appropriate documentation.
Provide support, advice, information and education to ward staff and multidisciplinary teams to facilitate a safe and timely discharge back to the community.
Undertake all aspect of clinical duties as an autonomous practitioner.
Ability to work with all members of the MDT in obtaining discharge information including but not exclusive to: care plans, discharge destination and estimated date of discharge for all inpatients.
Educate patients/families and enable them to make informed choices in the planning of the discharge.
Communicate your knowledge of the legal framework under the Care Act 2014 and duties relating to hospital discharge and other legislation relating to discharge planning.
Gain high levels of trust and confidence with both patients and their carers through exceptional communication and high-quality discharge plans to deliver a smooth transition into our services.
Liaise with referrer / consultant to gain approval to commence HomeLink Healthcare Services and projected care plan and discharge goals including frequency, duration, type of clinical interventions based on the clinical assessment develop a personalised care plan and agree discharge goals that addresses all care needs and is within the scope of the agreed operational manual.
