A fantastic opportunity has arisen for an efficient, organised and experienced Care Facilitators to join a highly reputable, established and rapidly expanding Domiciliary Care Business and PA service, We have a part time position for our Extra Care Scheme in Billingham, This is a fantastic opportunity for those individuals who have been a previous Care Coordinator, or looking to progress further that have been Senior or a Field Care Supervisor.

The role of the Care Facilitator is to support the delivery of the highest quality care support services by line managing care workers including some field base work carrying out a range of functions in the field, including the assessment and review of individual services and the practical supervision, instruction and support of care worker.

Typical Duties:

- Line manage and supervise Care Workers and PA's
- Organise and coordinate weekly, rotas and Care Services via our electronic care management system.
- Implementing Care Plans, Risk Assessments and customers reviews
- Ensure that the care worker rotas are organised and sufficiently well planned taking into account travel time and routes
- Plan and allocate care assignments to appropriate care workers, with due regard for the particular needs of the individual and the skills and attributes of the worker
- Liaise as necessary with partner professionals and organisations (e.g. social workers, GPS, nurses) in respect of developing and delivering holistic, person-centred packages of care
- Dealing with queries from clients and families in a calm and professional manner.
- Carry our spot checks and observations on care staff to ensure compliance and safety is maintained
- Managing Medication systems and carrying out daily Emar audits via our bespoke care management system
- Carry out on-call and response duties outside normal office hours as agreed

Person Specification:

- Experience of delivering and managing care support services.
- Understanding of principles of good care.
- Able to prioritise, particularly under pressure.
- Flexible; willing and able to work outside normal hours wen requested as part of a on call rota
- Completed a NVQ Level 3 qualification within social carer
- Completed a Level 3 Medication Qualification
- Organised and able to manage effectively in a fast paced working environment.
- Completed care plans and risk assessments to a good standard
- Have experience working within social care or similar role
