Berkshire NHS Foundation Trust are currently seeking a Band 6 Health Visitor to join their team on a temporary basis for the next 12 weeks. Working hours for this role with be 9am - 5pm Monday - Friday.

**About the Trust**

In this role you will be located in and working across an area with all the benefits of country living with the convenience of big city life.
**What you’ll be responsible for**
- Offer a comprehensive health visiting service in accordance with Berkshire Healthcare Foundation Trust (BHFT) policy and guidelines and ‘The Healthy Child Programme 0-5 years.
- Assess child and family needs, using the appropriate assessment and risk assessment tools.
- Ensure that at each contact with a child an assessment of the health and developmental need is conducted.
- Develop and implement outcome focused care plans and evaluate care given, in partnership with clients
The role will involve, but not be limited to you performing the above.
**You’ll have the following qualification/experience**:
- A minimum of 6 months paid experience as a Health Visitor for a UK hospital within the last 3 years
- Comply with NMC Professional Registration and keep up to date with current nursing practices
- Ensure accurate records are kept in accordance with CHS policy and NMC guidelines
- Undertake regular mandatory training*
- Full UK driving licence and own vehicle.
**Depending on the Trust, you may require variable additional certification and skills.**
