A dynamic self-starter is required in this exciting new role, supporting homeless clients to access wellbeing opportunities. Funded by Public Health and reporting to Housing Needs Teams across East Sussex, you will be working with the Council, Public Health, key partner agencies, local charities and commissioned providers, to bridge the gap between health and housing for some of our most vulnerable clients.

As well as managing a caseload of wellbeing clients, you will act as the point of contact for Wellbeing and Employment queries for the Housing Needs Team, and be responsible for coordinating pathways, working collaboratively with key partners to improve outcomes for homeless applicants, including mental health and wellbeing, social isolation and engagement, physical health, substance misuse, training, employment and of course, sustainable long term accommodation.

You must have proven experience of successful multi-agency development work, preferably in a homeless or public health context; an excellent comprehension of the complex needs of homeless applicants, as well as the available local resources within health and housing; a good working knowledge of social prescribing and ability to communicate effectively at all levels.

Hours and days may vary according to the needs of the project, but will be a mix of working from home, with outreach and in-reach visits in targeted areas across Eastbourne and Lewes, Wealden, Rother or Hastings as required by the service.
