The role requires you to have a registered professional qualification in health or social care with significant experience in leadership roles within Health or social care settings and experience of developing governance systems to oversee quality and safety of service provision.

You will be working as part of an team of 5 Integrated Neighbourhood Teams across the borough and this team plays a crucial role in this. You'll be leading health, primary care and social care professionals who will be working together as one team. You'll help them work collaboratively with voluntary and community groups to empower people to take control of their care and improve wellbeing. It's 'devolution in action': the people of Bury taking control of the way their own health and care services best meet their needs.

**Main duties of the job**

**Leading one of the Five Neighbourhood teams you will**:

- Be the senior strategic lead for the delivery and development of Integrated Health and Social Care teams which are neighbourhood based aligned geographically to GP practices.
- Lead in developing, delivering, and evaluating high-quality person-centred services in conjunction with the associated health and social care teams meeting the neighbourhood and integrated delivery collaborative objectives.
- Realise the INT ambition at neighbourhood level as part of the Let’s Do It Strategy
- Utilise an Organisational Development approach to bring together staff from multi professional and multi agencies to develop a high performing, cohesive Neighbourhood Team with the right values, behaviours, and competencies.
- Develop and manage key relationships with GP practices and other key stakeholders to deliver the vision for neighbourhood.
- Ensure the Active Case Management function of the Multidisciplinary team is delivered to prevent admission to hospital and reduce demand on Primary and social care. This requires in depth clinical and professional knowledge of how-to co-ordinate clinical and social care to highly complex individuals and their families who experience a wide range of co-morbidities and are affected by the wider determinants of health including adult safeguarding issues, social isolation, severe mental health issues as well as others.
