We are looking to recruit an established band 7 Advanced Clinical Practitioner (ACP / ANP) to join our Hepatology Medicine Service at University Hospital Southampton NHS Foundation Trust (UHS). This is an exciting opportunity for an ACP to gain Hepatology experience in a well-established centre that puts patients at the heart of all we do.

ABOUT THE TEAM

The Hepatology team is expanding. We now have 7 consultants, a large team of clinical nurse specialists, a specialist dietician and pharmacist and we are hoping to add to the existing Hepatology Advanced Clinical Practitioner team with a long term goal of creating nurse consultants. The trainee Advanced Clinical Practitioners (tACP) / Advanced Clinical Practitioners (ACP) are the 1ststep towards this. ACP’s are central to the team and provide a key role in the care of day case and inpatients.

The tACP’s/ACP’s are pivotal in supporting the patient pathway in order to reduce length of stay and avoid hospital re-admission, whilst seeking innovative ways to support our most complex patients. Supported independent practice is the aim, with the ACP being the pivotal person in the longterm care of our decompensated cirrhotic patients, on the wards, in clinic, as part of the ascitic drain service and also linking with liver transplantation, and end of life care services.

If successful, you will play a key part in delivering specialist Hepatology training and education to ward based multi-professional teams across the Trust.

We have the training facilities to support Practitioners to facilitate procedures such as; Practitioner’s will be encouraged and supported to undertake quality improvement / research projects to further improve patient care and service delivery.

Your Responsibilities

What you’ll do

As a Trainee Advanced Clinical Practitioner you are expected to work in conjunction with the 4 pillars of advanced Practice (HEE Framework Oct 2017)
- Expert clinical practice
- Leadership and management, in order to improve and transform services.
- Education and Development of self and others.

You will work as an independent Practitioner under the supervision of the Designated Medical Practitioner (DMP).

You will practice clinically within agreed care protocols; these will be monitored by the Designated Medical Lead.

This role involves undertaking competency based patient assessment and examination in order to formulate a management plan for specific groups of patients admitted to UHS for diagnosis and / or treatments.

The post holder will work across professional, departmental and Trust boundaries as required to develop and support patient-centred seamless care, with particular responsibility for participating in practitioner-led care initiatives.
