Pontypridd, United Kingdom Cwm Taf Morgannwg University Health Board Full time

An exciting opportunity has arisen to work in a newly developed service within Cwm Taf Morgannwg. The Specialist Mental Health and Substance Misuse Housing Outreach service support those in temporary accommodation or hostel in accessing the appropriate health services.

The aim is to deliver evidence based holistic mental health care to people within the context of an integrated multi-disciplinary, multi-agency specialist service.

We are looking for a creative practitioner with knowledge of working with and supporting complex individuals and advising accordingly on mental health or substance misuse related issues.

The team currently consists of Local Authority, Health and Third Sector staff. Including Trauma Informed Counsellors, Nursing staff and Drug Services.

The service runs Monday to Friday 9am to 5pm and covers all of CTM. Due to the expansion of the service our aim is to look at locality based working but this would need to flexible to cover the needs of the service i.e sickness or where a high level of referrals are received.

The post holder will provide support and intervention for individuals through holistic person centred assessment, treatment and evaluation of care within a multi-agency team approach.

They will demonstrate safe, clinical decision-making and expert care and support for people within the community whether this be at home or temporary accommodation. They will take responsibility for a caseload of clients as referred to the service in addition to provide specialist advice to external agencies and carry out appropriate teaching (within hostel settings).

The post holder will work collaboratively with the wider multi-disciplinary team to meet the needs of the people within the community. This will generally involve working with adults who have health needs due to complications of drug, alcohol use and/or mental ill health and would support transitional arrangements for people requiring primary and secondary health support.

As this is a newly established service the post holder will be required to contribute to the continuing promotion, improvement and development of the service through networking, representing the service at all times.

The post holder will be required to deputise for Band 7 nurses occasionally. This will include supervision of Band 3 HCSW's, attendance at various meetings and ensuring availability to offer advice and support to housing providers.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
