Discharge Support Worker

The discharge Support worker (DSW) will work closely with the ward manager and the ward multidisciplinary team, supporting all aspects of discharge planning at ward level. The DSW will be allocated to a specific ward on rotation and will develop close working relations with the ward staff.
The DSW will Collect and record comprehensive information on transfers of care and to share this with locally agreed social services, health and community partners, as identified by the discharge team.
Meet daily with the discharge team to record comprehensive and accurate transfer of care information. This information will be shared with internal and external partners using an agreed mode of information sharing as set by clinical governance guidance and hospital policy.
Provide administrative support to the discharge team. This will involve:
Arranging meetings (venue, time, invitations)
Collation of paperwork as directed by the d/c team
When clinical evidence is required for assessments, photocopying and collating documentation as directed
Taking minutes and distributing meeting minutes and notes

To update social services and health care partners on the progress of discharges and all delegated actions, and escalate issues that cannot be resolved within agreed timeframes
Update and change and amend any records or information as directed by Clinical Lead Discharge Practitioner. This will included updating and maintaining service databases and filing systems.
Prioritise the daily workload and complete tasks within agreed timescales.
Act as a key point of contact for the discharge team with regards to transfers of care. This includes dealing with telephone queries from patients, families or professionals, processing referrals in accordance with standard operating procedures and undertaking other administrative tasks such as filing, faxing and data entry.
Undertake GP and community liaison calls to inform partners of patient admissions and obtain information about patients that may help prepare for discharge and to inform the the multi-disciplinary team in a timely manner.
Assist in creating lists of patients with long lengths of stay and those who are no longer meeting criteria to reside and distribute accordingly.
Ensure the notes of any meetings are typed, verified as correct and sent to all persons on the distribution list.
Use Trust IT systems and specialise databases to ensue relevant and accurate information is recorded, and will liaise with IT regarding any equipment repair, training issues are registered and resolved
Order office supplies and other equipment as directed
