**Key Accountabilities**: 1.Provide and deliver a high standard of compassionate, safe and effective care to patients and support them to improve and maintain their emotional, mental, physical, behavioural and spiritual health and well-being 2.Promote comfort and well-being by ensuring that all aspects of the patients needs are met, and follow care plans as directed within the patients Systmone records. 3.Carry out clinical observations on patients, monitor their condition and identify/report any changes in the physical, mental, emotional and spiritual condition to the registered nurses or members of the MDT. 4.Assist in and perform clinical skills within specific competency boundaries related to patient care as directed by Registered Nurses or Hospice Doctors. This could include venepuncture, ECG recording, measuring and interpreting blood glucose levels, recording and interpreting vital signs, nutrition and hydration 5.Assist patients in maintaining their personal hygiene, grooming, dressing and oral care needs, ensuring privacy and dignity at all times.

6.Assist in the safe moving and handling of all patients utilising appropriate equipment. 7.Support patients after death with last offices, ensuring patients and families wishes are supported 8.Ensure all clinical records are kept up to date and relevant to patient care. 9.Work with all clinical professionals and engage with other agencies and community teams to ensure a safe personal approach for patients. 10.Work with Registered nurses with the assessment of patient and relatives needs and support discharge planning.

11.Engage with clinical supervision to reflect on experiences, develop clinical skills, strategies for handling specific circumstances and enhance your emotional intelligence. 12.Communicate and collaborate effectively across the wider multi-disciplinary team, patients, carers families and friends whilst demonstrating compassion and empathy.
