To be responsible for the data input into clinical systems that support flow and timely discharge of patients.
To assist with simple discharges and support the IDT team by using effective co-ordination and communication skills to enhance the discharge process; including completion of CHC, Fast Track and Home First referrals for authorisation by a Registered Practitioner.

In supporting the flow of the ward, the post will entail non-clinical tasks and administrative tasks under the supervision of the Nurse in Charge.
The post-holder will be required to provide a link between the ward and internal services such as Site Managers, Matrons and the discharge lounge to achieve Trust targets, providing administrative assistance and ensuring effective communication between Multi-disciplinary Team members.
