JOB DETAILS
JOB TITLE: Mental Health Practitioner
BAND: 6
HOURS: 37.5 (During office hours Monday - Friday but can accommodate part time hours.
DEPARTMENT: Adult Mental Health
LOCATION: Stockport
REPORTS TO: Manager
- As a mental health practitioner, you will be required to carry out CPA & Risk assessments for patients.
- You will also be required to carry out a care coordination role
- Some work from home depending on individuals' confidence
- Home visits to service users (PPE given)

As well as the above duties, you will also be required to carry out duties such as
- Being shift coordinator, nurse in charge
- Administering medication,
- Admissions and discharges.
- Patient care
