An exciting opportunity has arisen to be part of the Families and Integrated Community Care Division supporting the implementation and operational management of the Fylde Coast Virtual Ward Provision

The Virtual Ward Lead role will be responsible for providing leadership to ensure that safe and effective care is delivered, within the resources available and that care is organised to meet the needs of the patient whilst on the Virtual Ward, enhancing their experience. Working together with Clinical Leads and partners the role will also play a proactive role to build strong relationships between those operationally delivering care across the Virtual ward pathway from referral to discharge.

You will be part of a friendly supportive management team who work hard to maintain a patient focus in everything we do. We are seeking motivated, proactive individuals that help move services forward in partnership with primary care, secondary care, and our social care colleagues.

- To be the operational manager for the Virtual Ward beds, responsible for the coordination of teams delivering care and ensuring that priorities are met.
- To initiate and be influential in the creation of care pathways and the development and update of standard operating procedures for people on the virtual ward and following discharge from the ward
- To ensure the achievement of key performance indicators for Virtual Ward
- To ensure that accurate case recording occurs regarding all aspects of the work undertaken with service users, this includes electronic record keeping and MDT meeting recording.
- To be responsive to local and national initiatives and guidance to ensure service delivery is integrated, patient focused and meets the changing needs of the local populations
- To ensure strong links are maintained with the community and inpatient teams
- To manage the Virtual Ward team operational budget
- To monitor the spend along with finance colleagues in line with budget

In order to meet the needs of the role and service, you will be regularly required to work outside your normal place of work. The Trust reserves the right to change your normal place of work to any other location within the Trust.

**Responsible for**: Virtual Ward Beds across Fylde Coast

**Key Relationships**: Cross Divisional partnership working,multi-agency colleagues including but not limited to; Fylde Coast Medical Service (FCMS), Trinity Hospice, Local Authorities, GPs, ICB

JOB PURPOSE

Ensure patients are assessed, have care plans that are implemented and evaluated in accordance with the Trust’s, NMC and HCPC requirements and evaluate patient outcomes to ensure safe and effective care.
