**1.1 Job Summary**

Bexley Health Neighbourhood Care (BHNC) is a GP Federation supporting the 22 Bexley Practices / 4 PCNs who are working collaboratively to enhance the health and wellbeing of Bexley residents, covering over 246,000 patients.

This is an exciting opportunity for someone looking to make an impact on the lives of patients living within Bexley and other South London boroughs, to support with multiple projects provided by BHNC within the preventative services team. This team delivers a variety of NHS services including structured diabetes education programmes, the South London Diabetes Book and Learn service (DBL), NHS Health Checks and Stop Smoking service among others. These services support patients to manage their own health and wellbeing, with the aim of preventing hospital admission and to care for patients within the primary care setting.

We are looking for a qualified NCSCT trained, Stop Smoking advisor to join the team and work alongside our current advisor, to support with clinics and promotion of the service. This role will work closely with our primary care colleagues, public health and the Stop Smoking team to support the development and implementation of Bexley Stop Smoking Service. A key element of this role is to provide specialist Stop Smoking treatment and support directly to patients at a number of venues in Bexley, as well as on the phone appointments, to reduce smoking prevalence in the borough.

BHNC will make a positive impact on medical services in Bexley, by working closely with the CCG, local NHS trusts, local providers and patient groups, to improve the delivery of healthcare to the local population.

**1.3 Key Responsibilities**

**Duties**
- To work towards achieving the PHOF relating to smoking cessation. To ensure practice of stop smoking support is informed by the latest evidence base as contained in the PHE Service and Delivery Guidance 2014 and is in line with NICE guidance.
- To ensure that all health and social care providers are identifying appropriate smokers and referring to stop smoking support. To actively seek out opportunities to maximise referrals to the Service.
- To develop and strengthen links with primary care and other local partner providers to ensure that effective referral pathways are in place for smokers.
- Support the Public Health smoking cessation agenda by closely working with the Preventative Services Manager and others involved at all levels of smoking cessation to enable the London Borough of Bexley to reduce smoking prevalence in Bexley.
- To facilitate high quality specialist stop smoking support on an individual or group basis to smokers living or working in the London Borough of Bexley in order to contribute towards meeting or surpassing smoking cessation targets.
- This will involve assessing individuals referred to the specialist service to ascertain motivation to quit and providing specialist stop smoking advice, pharmacological advice and carbon monoxide monitoring.
- Normal working days will be Monday to Friday but a flexible approach to work will be needed as the post holder will be required to provide cover for other Stop Smoking Advisors’ clinics from time to time. This may mean working later on some days of the week to accommodate service delivery as well as occasional weekend work or evenings.
- Plan and deliver training programmes to health professionals so that they can provide high quality interventions to smokers in line with NICE guidance and NCSCT.
- Collect and monitor statistics by inputting data electronically and in a timely manner onto the Quit Manager programme so that progress towards targets can be accurately analysed.
- Ensure all referrals to the Service are acknowledged and booked onto the programme.
