**Job Title**:Senior Health Care Assistant - NVQ level 3 or Equivalent and Above**

**Hours: Day/Night Shifts**

**Salary: Negotiable salary/ hours per week**

**JOB PURPOSE**:
Provides defined and delegated aspects of patient care under the supervision of Manager and supports the Senior Staff team by undertaking a range of support functions. To support senior staff and the management to implement necessary quality indicators to improve delivery of care in accordance with company ethos.

**SCOPE**:
Undertakes clearly defined and controlled tasks for named patients referred by the manager. You will work in accordance with specified local and national protocols and guidelines.

**MAIN DUTIES AND RESPONSIBILITIES**:

- To ensure that standards, systems and methodologies laid down by the management are implemented at all times.
- To communicate on a regular basis with the management on events that occurred in his/her absences and vice versa
- To assist in provision of a planned care, recorded and reviewed at regular intervals for individual service user. This would be done personally or with the team of carers.
- Undertake all duties that are also necessary to promote and assist in personal hygiene, mobility and toileting in conjunction with care assistants.
- Monitor the quality of care delivered by the team whilst on duty, report issues arising from working with the carer. Act as a role model for all staff.
- Support and encourage the caring team to perform in line with expectations.
- Carry out supervision of all caring team regularly and record it.
- To promote the general physical appearance of service users at all times. If they require new items, please ensure that the family has been contacted and under MCA request replacement. Keep a log of all conversation on CMS.
- Acts as a chaperone for patients as required, preserving the patient's dignity.
- Provides relevant health information to patients and carers within defined protocols.
- At all times observes infection control procedures to protect self, patients and colleagues.
- Assists in maintaining a safe working environment for self, patients, carers and the multi-disciplinary team, alerting other members of the team to issues of quality and risk identified in work with patients.
- Conveys relevant information and receives feedback from the manager / GP’s
- Undertake and organise staff review meeting with manger. Make necessary records.
- **Hands on the floor during the morning shift, also be available for District Nurses and GPS visits.**:

- **Share on-call responsibility equally with the manager within the job remit.**:

- **Over time will be offered as a senior staff on regular basis at usual hourly rate to cover absences.**

**ORGANISATIONAL**
- Practices and promotes confidentiality at all times
- Maintains accurate records of patient contacts, providing clear written and verbal information as required by the Senior Staff.
- Actively participates in the smooth running of the service
- May be expected to undertake some audits to demonstrate effectiveness.

**SYSTEMS AND EQUIPMENT**:

- Recording all information on CMS as required.
- Actively participates in the care and maintenance of equipment, demonstrating economy in the use of supplies and resources.

**DECISIONS AND JUDGEMENTS**:

- Works at all times within the established policies, standards and guidelines of the Company.
- Undertakes clearly defined duties that have been agreed and taught.
- Refers back to a staff nurse any patient where an informed clinical judgement is required before proceeding with treatment.

**COMMUNICATIONS AND RELATIONSHIPS**:

- Actively communicates with all colleagues, patients, relatives, carers, visitors and other agencies in a professional manner at all times
- Communicates effectively with patients and carers, recognising patients' rights and responsibilities.
- Works as member of the caring team, seeking advice, guidance and support from other members of the team
- Communicates concerns identified in patient to others in the team.
- Works under the supervision of a staff nurse or the management, who ensures that skills are maintained and developed to meet the needs of the post.

**PERSONAL DEVELOPMENT**
- Actively participates in the In-Service training opportunities available
- Maintains responsibility for own personal development under supervision and guidance from qualified staff
- Demonstrates a positive attitude to suggested changes and development in company through the introduction of evidence-based practice
- Required to undertake annual Mandatory training, e.g. Basic Life Support, Moving & Handling, Fire

**MOST CHALLENGING/DIFFICULT PART OF THE JOB**:
The demands of this post require a person who works well under pressure and who is enthusiastic, self-motivated and who can remain calm in sometimes stressful situations.
