We are looking to recruit a Community Connector to support our busy team. Working closely with GP’s and health colleagues, Social Care Services, and the Voluntary Sector within our 4 Primary Care Neighbourhoods, you will provide personalised support to residents to improve their health and well-being, and address wider issues affecting them.

The role is all about empowering people to take control of their own health and wellbeing by giving them time to focus on what matters to them. You will build trusting relationships, create a shared personalised support plan and connect them to community groups, voluntary organisations and services in their neighbourhood.

By working with partners across Blackburn with Darwen as part of a Social Prescribing Alliance, you will ensure people can access the right support for them. You will be a key figure in ensuring that community groups and voluntary organisations are able to thrive in their neighbourhoods.

A crucial aspect of the role is to help people to work on their wider health and wellbeing; addressing not just health access and outcomes but also the wider determinants of their health, such as debt, social isolation, poor housing and physical inactivity. The role will support lifestyle improvements and low-level mental health concerns by increasing people’s active involvement in their local communities. This approach particularly helps people with long-term conditions including mental health, lonely and/or isolated residents, and complex social needs affecting wellbeing.

Excellent interpersonal skills are integral in supporting people, communities and local organisations to help them care for each other, and you should have experience of working positively with people facing complex social and emotional challenges.
