Surrey & Borders Partnership NHS Foundation Trust are currently seeking a Band 7 Mental Health Nurse to join their team until May 2023. This role is full time working Monday to Friday 9am to 5pm.

Rated 'Good' in their most recent CQC report, Surrey and Borders Partnership NHS Foundation Trust provides hospital-based and community services for people of all ages with mental ill-health and learning disabilities in Surrey and North East Hampshire aswell as providing drug and alcohol services in Surrey and Brighton. **What you will be responsible for**
- Contribute to delivering and co-developing the primary care mental health service model and care pathways for the provision of integrated physical and mental health care in Primary Care Networks.
- Accept referrals direct from primary care and promote early assessment, treatment and ensure robust relationships and links with other community based services.
- Work predominantly with adults (18+)with SMI whose needs are best met within primary care and whose difficulties are best understood within a biopsychosocial model.
- Work closely with colleagues in specialist community mental health services to ensure smooth transitions between teams and services and facilitate an 'easy in, easy out' approach to improve access to evidence based interventions
- Facilitate mental health and assets based assessments in Primary Care and support the Primary Care Team in accessing appropriate level of services.

The role will involve, but not limited to you performing the above.

**You will have the following qualification/experience**:

- A minimum of 6 months paid experience in a similar role for a UK hospital environment within the last 3 years.
- Comply with NMC Professional Registration and keep up to date with current nursing practices
- Ensure accurate records are kept in accordance with CHS policy and NMC guidelines
- Undertake regular mandatory training*

**Depending on the Trust, you may require variable additional certification and skills.**
